I/We the parents of the above named candidate for a position on a Rock Island Girls Softball League Team, hereby give my/our approval to her participation including transportation to and from the activities, and I/we do hereby waive, release, absolve, indemnify, and agree to hold harmless the Rock Island Girls Softball League, the Rock Island Parks and Recreation Department, the Organizers, Sponsors, Supervisors, Participants, and Persons transporting my/our daughter to or from activities or for any calm arsing out of an injury to my/our daughter. A FEE OF $35 MUST ACCOMPANY THIS REGISTRATION. ALL GIRLS WILL BE REQUIRED TO PARTICIPATE IN A MONEY RAISING PROJECT (FUNDRAISING MONEY IS NON-REFUNDABLE).
